LASA — look-alike / sound-alike names

Claim

Look-alike and sound-alike (LASA) drug names are a known source of dispensing errors. ISMP publishes a List of Confused Drug Names (including pairs used with tall man lettering) to highlight pairs that have been confused in practice.

If two names look or sound close, slow down: purpose on the Rx, tall man lettering, barcode. ISMP keeps the confused-names list current.
