Days supply — Qty ÷ (dose × frequency)

Claim

Days supply is calculated as total quantity dispensed divided by the amount used per day (dose × frequency), then checked against fill rules and any day-supply limits on the claim. Wrong days supply is a common PTCE miss and a real refill dispute.
